Clinic Appointments

To schedule a visit with Dr. Kachmann in the Holistic Pain Management Clinic call 436-2416 ext 2108 . Patients are seen by appointment only. All clinic visits are scheduled at Lutheran Hospital Cancer Resource Center Clinic.

For your first visit, please bring the following items:

• Insurance card
• Referral from your care physician (if needed)
• Results of diagnostics testing (MRIs, CAT scans, etc.)
• List of prescriptions and medical conditions
• Medical records
